Our care

Care that comes to you

More than medical care: a team beside you, wherever you live, working from one plan.

Who this care is for

We care for adults living with a serious, life-limiting illness who have chosen comfort over treatments aimed at cure. Common diagnoses include cancer, heart failure, lung disease, dementia, kidney or liver disease, stroke, and ALS. A physician confirms that the illness is expected to limit life to months rather than years if it runs its natural course. Many families tell us they wish they had called earlier: the sooner we're involved, the more we can do.

You can keep your own doctor. If he or she is agreeable, your doctor continues as your attending physician, and we work alongside them, keep them informed, and carry out the plan together.

Your care team

Your care team brings

Every person in our care has a written plan, made with the family and revised as things change. Visits increase as needs increase. The people who carry it out:

Registered nurse

Your case manager. Manages pain and symptoms, teaches the family, coordinates everything, and visits more often as needs increase.

Aides, seven days a week

Bathing, dressing, turning, skin care, and the ordinary work of a hard day. Aide visits are scheduled seven days a week, including weekends and holidays, at the frequency your plan calls for.

Social worker

The practical and the unspoken: paperwork, benefits, family conversations, caregiver strain, planning for what comes next.

Chaplain

Spiritual care, if interested, in your tradition. Presence, ritual, reconciliation, and blessing, on your terms.

Physician oversight

Our medical director reviews every plan of care and is available to your attending physician for symptom questions.

Trained volunteers

Companionship, vigil sitting, life-story recording, errands, and a break for the caregiver.

When more is needed

Most care is routine care at home. When a day calls for more, the plan already has a path.

  1. Routine care at home

The everyday pattern: scheduled visits from the nurse, aide, social worker, and chaplain, with a nurse a phone call away at any hour. This is where most care happens.

  2. Crisis care at home

When symptoms flare and need close attention, such as a pain crisis, severe breathlessness, or agitation, a nurse can stay in the home for extended hours until things settle.

  3. Respite stays

Caregivers wear out. We can arrange a short stay of up to five days in a contracted facility so the family can rest, travel, or simply sleep.

  4. Intensive comfort care in a facility

When symptoms cannot be managed at home, we have a path to a contracted facility for round-the-clock symptom control, with the goal of returning home when it's safe.

Where we care

We come to you: your home, a family member's home, or a care community. We serve Carmel, Westfield, Fishers, Noblesville, Zionsville, Greenfield, Anderson, Pendleton, the north side of Indianapolis, and surrounding communities in Hamilton, Boone, Hancock, Madison, and Marion counties.

Paying for care

Covered services, medications, equipment, and supplies related to the illness are provided at no charge to the patient. We will help you understand how Medicare, Medicaid, or other coverage applies to your situation before anything begins. We provide care without regard to race, color, national origin, age, disability, sex, religion, or ability to pay.
