Serving Carmel and the Indianapolis north metro (317) 558-9305  A nurse answers, day and night.

For healthcare professionals

Let's meet before day one.

We're building Oak Grove Pathways to be the agency you're glad to hand a family to: responsive, clinically sound, and easy to reach. Here's how we'll work with you.

What you can expect from us

  • One call, one nurse. Referrals and clinical questions reach a nurse directly at (317) 558-9305, 24 hours a day.
  • Prompt response. Same-day contact with the family on every referral; evaluation and admission as quickly as the family is ready, including evenings and weekends.
  • Your patient stays your patient. Attending physicians remain the attending. We keep you informed, send visit summaries, and call before we change anything you'd want to know about.
  • Symptom expertise at home, with a defined path to crisis care in the home, respite stays, and facility-based intensive comfort care when needed.
  • Small caseloads so your patients see the same clinicians visit after visit.

Who we serve

Adults with a life-limiting illness (cancer, heart failure, COPD and other lung disease, dementia, renal and liver disease, stroke, ALS and other neurological conditions, and general decline) whose goals have shifted to comfort, and for whom the attending physician expects a prognosis measured in months if the illness runs its natural course. We care for patients in private homes, assisted living, memory care, and skilled nursing facilities across Carmel, Westfield, Fishers, Noblesville, Zionsville, Greenfield, Anderson, Pendleton, the north side of Indianapolis, and surrounding communities.

What a referral will need

A complete referral is short:

  • Patient name, date of birth, and current location (home or facility, with address)
  • Primary diagnosis and pertinent history; recent notes, medication list, and any advance directives or code status
  • Attending physician name and contact, and whether they will remain attending
  • Family or representative contact
  • Your name, role, and best callback number

To refer a patient, call (317) 558-9305 or send us a message.

Partnering with facilities

Assisted living, memory care, and skilled nursing communities: we'd welcome a conversation about how we support your residents and your staff, including shared care planning, staff education on comfort care and symptom recognition, after-hours coverage that reduces avoidable transfers, and clear communication with your nursing leadership. We are also arranging respite and inpatient-level contracts and would be glad to talk if your community provides those levels of care.

Physicians

We welcome conversations with physicians, nurse practitioners, and physician assistants interested in serving as attending providers for their own patients in our care, or in joining our interdisciplinary team meetings as a consulting voice.
